2016/2017 Summary of Switch to EPC-
 
Delta Dental- open enrollment form to fill out-
This MUST be completed by ALL benefit eligible employees, even if you are waiving coverage vision or dental coverage. Return the completed form to Todd or Bonnie.
  • Everyone will get a new ID card for Delta Dental in the middle of December.  This also will be effective 1/1/17, so don’t throw away the old one until Dental Care Plus expires on 12/31/16. 
  • Dental coverage changing from Dental Care Plus to Delta Dental of Ohio
  • Delta Dental has matched the benefits provided by Dental Care Plus – attached is a benefit summary
    • To search for participating providers, please visit www.deltadentaloh.com
    • Select “Premier” or “PPO” networks
    • No login necessary!
  • All members will receive ID cards in the mail

  • Optional Vision coverage changing from Avesis to Vision Service Plan (VSP)
  • VSP is offering two (2) voluntary vision plans – attached are both benefit summaries
    • To search for participating providers, please visit www.vsp.com
    • There’s only one network, which eliminates confusion
    • No login necessary!
  • Members will not receive ID cards in the mail; however, after open enrollment, members can login and download/print a temporary ID card
    • Monthly Plan Costs: Option #1 – Single:  $7.25 / Family:  $16.90
      Option #2 – Single:  $9.21 / Family:  $21.55
 
Anthem- no paperwork to fill out
  • In the Middle of December please be on the lookout for new Insurance cards
  • Everyone that has the District’s health insurance will get a new card.  This will have new number, and you will have to give your health care professionals the new card to have claims paid effective 1/1/17. Your current Anthem card will expire 12/31/16.
 
Prescription Drug Coverage changing from Anthem Express Scripts to CVS Caremark- no paperwork to fill out
  • This change is effective January 1, 2017
  • ID CARDS: Members will receive a separate ID card from CVS Caremark (for Prescription Drugs)If you try to use the Anthem card at the pharmacy, Anthem will say you do not have coverage as of January 1, 2017. Please use your CVS card as of January 1, 2017.
  • Please check your mail for these items. The prescription card will be in an envelope from Southwestern OH EPC-CCIC. This is not junk mail!
 
Chard Snyder Flexible Spending Account- one form to fill out if you choose to open a flexible spending account
  • If you are selecting to open a flexible spending account through Chard Snyder see the attached forms! This is an optional program and must be returned by November 18!
  • Chard Snyder (Flexible Spending) - Forms and information are here. The maximum amount for 2017 Calendar year will be $2,500.
